Diagnose your PMS/PMDD using the PMS symptom record chart



Charting PMS symptoms is one of the best ways to get an idea of when, how and what kind of PMS symptoms you get. Recording your symptoms will also help your doctor to be able to diagnose your PMS and to provide you with adequate PMS treatment. Your doctor would need at least 3 filled PMS charts to be able to have a definite diagnosis regarding the severity of your PMS or PMDD. Remember that for a correct diagnosis of PMS the following 3 criteria need to be fulfilled;
  • Symptoms must have been present every month for at least the previous two menstrual cycles.
  • Symptoms must be present before menstruation and cannot start before ovulation (roughly 14 days before menstruation)
  • There must be a complete absence of the symptoms after menstruation for a minimum of 7 days.

    The chart is numbered by the days of the month. It is important that you record your PMS symptoms daily as described in the PMS chart.

  • Fill out the month and year at the top of the form.
  • Color the boxes under each day according to the severity of your symptoms. (no colour for no symptoms, green for mild symptoms, orange for moderate and red for severe).
  • The chart is just numbered by the days of the month (do not confuse these numbers with the days of your cycle). You just have to remember to circle the dates when you are menstruating so that both you and your doctor will be able to make the connection between the symptoms and menstruation.
